Applications are processed on a first come, first served basis. There is no guarantee of placement. There are NO REFUNDS unless a player cannot be placed by the NYSL. All applications postmarked after the deadline will be wait-listed. Shinguards are required for all practices and games. The NYSL uniform is required for all games. Failure of the player or parent to abide by the published rules and principles of the league may make their player ineligible to play. Submitting this form for processing constitutes agreement to abide by league rules.
Player Release
I, the parent/guardian of the above-named player, acknowledge that soccer is a physically-demanding activity from which injury can result. I understand that soccer is a contact sport. I assume the risk and want my child to play. My child has no medical problems that would prevent him/her from participating in vigorous physical activity. In consideration of the player's participation in activities sponsored by the Nashua Youth Soccer League, I, for myself and the player and our respective heirs, administrators and successors, intending to be legally bound, hereby release, hold harmless and indemnify the Nashua Youth Soccer League, its coaches, officers, directors, employees, agents, volunteers and representatives from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player's participation in the activities sponsored by the Nashua Youth Soccer League.

I hereby authorize the Nashua Youth Soccer League (NYSL) to seek emergency medical treatment for my child in case of injury or occurrence of a medical problem which may occur during league sanctioned activities. I also agree to notify the NYSL, in writing, of an injury or other medical problem that may be considered related in any way to a league sanctioned activity NO LATER THAN 24 HOURS AFTER the specified activity occurred. I agree that NYSL shall not bear financial responsibilities for seeking emergency medical treatment for my child and I agree that I will be responsible for the payment to any health care agency providing this aid.

Philosophy: The NYSL is a recreational, non-profit, volunteer organization that promotes the introduction of soccer to children ages 5 to 19 in the Nashua area. Children with birthdates from 10/1/1989 through 9/30/2003 are eligable for the Fall 2008/Spring 2009 season. All children interested in playing soccer are encouraged to join regardless of experience or skill.

Enrollment: Before any child is permitted to play, a registration form must be completed, signed and returned along with a check to the above listed address on or before July 31, 2008. Due to limitations of playing fields and volunteers, players are accepted on a "first come, first served" basis. Late registrants will be "wait listed" and will be assigned teams only if space permits. Once all forms are processed, coaches obtained and fields assigned, you will receive a call notifying you of your child's assigned team, coach, practice times, etc. However, do not expect a call until approximately the first week in September..

Playing Equipment: All players are required to wear the NYSL uniform, which consists of a green and yellow reversible jersey and matching socks, during games. Uniform purchases are made through the league only and provisions are available on the registration form. In addition to the uniform, other mandatory equipment includes shin guards, mouth guards for all players with any orthodonture and safety straps for eyeglasses. Cleats are recommended but are not mandatory.

The NYSL depends solely on the help of volunteers. Without it the NYSL will be unable to continue. Please consider dontating your time to help make this a better organization.

Though your youngster is under the supervisory guidance of fine coaches during soccer practices and games, the most important people in their life are still you, their parents. We'd like to pass on a few tips, which will make soccer more enjoyable for everyone involved.
